Surgical methods of apical changes treatment

Apical changes (apical granuloma) arise as a result of dental pulp illness or as a result of not properly treated root canals. The name derives from the place where it is situated, namely around the apical resorption. Thereupon the root cyst may arise. In the RTG photograph it is visible as a round change join connected with the apical resorption, very often surrounded by the white environment.

Cysts has a tendencies to growth expansion that is, growing evenly in every direction destroying the bone around. As soon as possible the enucleation of a cyst is necessarily.

If the apical changes are not big we may try the endodontics treatment. This kind of treatment may fail in case of big changes or root cysts. Nevertheless tooth may be rescued thanks to the surgical method that is, apicectomy.

1. Apicectomy.

The surgical procedure here, is performed under the local unaesthetic. It consists of the cut and detachment of mucous membrane than cut of the apectomy, and very careful cut of a inflammatory changes. When the bone cavity is large we replace it with materials which speed up the wound healing. [remote bone regeneration]

Mucous membrane is stitched up and the surgical dressing is put on. After the surgery the patient takes the antibiotic - doze and kind depends on the degree of the inflammatory process. Stitches are taken off after 7-10 days after resection. In the case when one, out of many tooth roots, is obstructed and around the top of the root is showing symptoms of inflammation we may perform the hemisection treatment.

2. Root hemisection, root amputation

Hemisection consist of cutting the tooth in half and the root extraction. Hemisection is performed on lower molar teeth without coalesced roots. But those differences this treatment is very similar to resection. The rescued root after the wound healing will be used to prosthetic reconstruction.

Surgical extraction of the wisdom teeth

Surgical procedure of extraction of the wisdom tooth is performed under the local unaesthetic and if necessarily with antibiotic protection. This treatment consist of a mucous membrane cut above the wisdom tooth kept or partially kept in the bone and than detachment of the mucous membrane. The extraction of the tooth and stitching is the next step. The surgery takes usually 20-60 minutes. After seven days the stitches are taken off.
